Scheduling Appointments: While walk-in appointments are welcome, we recommend booking at least two weeks in advance to ensure we can accommodate your service requests. All appointments will require a valid credit card number on file upon scheduling. A driver’s license or other valid form of ID may be required upon check in.
Consultation Fee: The fee for your consultation with our providers are as listed below. Consultation fees may be applied to the cost of your services received at Skin Matrx on the same day of your consultation. You may need to come in for a new consultation if it’s been over 6 months since your initial consultation or if you are interested in new services. Consultation fee subject to change without notice.
Cancellations and Missed Appointments: We kindly request that you give us a minimum of 2 business days’ notice if you are unable to keep your appointment. Failure to do so will result in missed appointment fees as follows:
- Missed/Cancelled Consultation: $100 Fee
- Missed/Cancelled Treatment: Forfeit deposit, typically 50% of treatment cost.
- Prepaid Package: One session will be forfeited from the package in lieu of the missed
appointment fee.
You authorize the above-mentioned fees to be charged to your credit card on file and agree that said charges are final and non-disputable. If credit card charges are declined or disputed, future appointments cannot be booked until the fees are paid in full.
Late Appointments: We ask that you arrive on-time for your appointment. If you are more than 10 minutes late, you may be rescheduled, and the missed appointment fee will be applied. For all new clients, we kindly ask you arrive 10-15 minutes early.
Payments: Payment is due in full at the time of service for all office visits and services. We accept Visa, MasterCard, American Express, Discover, Care Credit, cashier’s checks, and cash for your convenience. There is a $45 charge for returned checks. Blanka Orloff, M.D., Inc. (dba Skin Matrx) is not responsible for errors in billing made by third party vendors such as Care Credit, Visa, MasterCard, American Express, and Discover.
Financing Options: As a convenience to our patients, we offer financing through CareCredit. By choosing to finance the cost of your treatment, you certify that you selected the credit program based on your own opinions and did not rely on any recommendations or advice from Skin Matrx or its staff.
Cosmetic Procedures: Payment for all cosmetic procedures will be collected prior to the procedure. Payment for cosmetic services are non-refundable after services are performed and are not covered by insurance. Skin Matrx does not accept or participate in insurance billing.
Treatment Prep: At the time of booking, a Skin Matrx representative will go over all of the necessary preparation needed for your appointment. Should you arrive unprepared for your appointment, scheduled appointment time will be reduced to accommodate the following client’s booking.
Procedure Scheduling: A 50% deposit is required to hold your procedure date. Procedure scheduling requires careful planning and coordination between our office.
Late Fees & Collections: Any account balances that remain unpaid for 30 days from the date of
service shall accrue interest at the rate of 15% per year, compounded monthly, and may be referred to a collection company and/or attorney. In the event this occurs, you will be liable for collections, costs, and all related expenses. Further, in the event any unpaid account balance is referred to any attorney for collections, you agree to be responsible for all costs and reasonable attorney’s fees incurred in connection therewith.
Products and Services: A professional consultation is required before initial dispensing of any professional product or receipt of any treatment or procedure. All sales are final; Skin Matrx does not accept returns or issue any refunds. Gift cards are non-refundable and non-redeemable for cash.
Packages: All packages expire one year from the date the initial session is first redeemed. We do not offer any refunds for prepaid packages.
Medical Photo Consent: By signing this form, you are authorizing Skin Matrx and its providers and staff members to take before & after pictures of the procedure(s) that will be performed on you. These pictures will only be used to determine the optimum outcome of services and/or treatment and maintained as part of your medical records. They will not be displayed elsewhere without your prior consent.
Right of Refusal: Skin Matrx reserves the right to refuse treatment and/or dismiss a patient at any time at its discretion. Not everyone will be a candidate for certain services or procedures, and it is at the full discretion of the physician to determine whether an individual is a suitable candidate for services.
Minor Patients: The parent or legal guardian of a minor patient (under 18 years of age) is responsible for providing payment for services rendered, as well as supervising the minor while at Skin Matrx.
No Result Guaranteed: Payments rendered are for the procedure(s) performed and not for any specific result. There can be no guarantee or warranty, expressed or implied, by anyone as to the actual results you may get. Results will vary and include the possibility of no results. Additional treatments may be necessary at an additional cost to achieve optimal outcomes.
Address Change: It is important that we have your correct address and contact information on file. Please advise us anytime there is any change to your address, telephone or other contact information.
Personal Property: Skin Matrx is not responsible for damage, theft or loss of personal property.
Animals & Pets: We do not allow any animals or pets at the clinic. Service animals specifically trained to aid a person with a disability are welcome.
Personal Electronic Devices: This is your time to relax. For the comfort of all other guests, we ask that you please refrain from using your cell phone or other personal electronic device while in the facility.
Gratuities: Gratuities are not included in our pricing. Though our medical professionals cannot accept gratuities, at your discretion our aesthetic and massage personnel may accept gratuities via cash or personal check. Gratuities cannot be accepted in the form of credit-card payment.

Referrals: We graciously accept all referrals at Skin Matrx. To show our appreciation, we’ll send you a $25 gift certificate when your referral completes service and mentions your name.
